Collecting participant-specific data in ESF projects

Projects funded by the European Social Fund, such as Momentum, are obligated to collect specific data from their participants. This binding obligation is based on the ESF Regulation 1304/2013 and the Common Provisions Regulation 1303/2013.

Project executors collect the data with participant-specific forms, and register the data to the ESF Person system. These data are essential for both allocating ESF support and studying its impacts. The data registered on the participants are not used or disclosed for any other purpose.

INSTRUCTIONS: FILLING AND SENDING YOUR PARTICIPANT FORM

1. Download our fillable participant-specific form  (You can get the free Adobe Acrobat Reader PDF software here.)

2. Digitally fill in and sign the document, and save it onto your computer  (instruction video at the end of this page)

3. Send the filled and signed form to us by using our secured e-mail service
